Transforming Information into Action: With the support of CDC and the National Diabetes Education Program (NDEP), NAWHO’s multi-level campaign of community and professional education seeks to increase awareness of and action on diabetes among Asian Americans and Pacific Islanders (AAPIs).The goal of NAWHO’s program is to decrease the incidence, morbidity and mortality of diabetes among AAPIs by organizing communities to create and implement innovative, culturally and linguistically appropriate diabetes intervention strategies, and by strengthening the cultural competence of healthcare systems. Through its diabetes program, NAWHO has convened four national diabetes professional education symposia, which involved 400 health care providers and public health educators. Also, NAWHO partnered with 11 community-based organizations in seven states to provide diabetes education to community members and cultural competency training to health care providers serving Asian Americans and Pacific Islanders. This campaign has reached 9,834 AAPI consumers living with diabetes or who are at high risk of diabetes through in-person interventions, and nearly 600 health care providers. NAWHO’s primary program activity is the coordination of diabetes education projects among community affiliates located across the country. Each affiliate implements localized, culturally appropriate programs designed to most effectively reach the AAPIs in their state and community. NAWHO provides technical assistance and support to its affiliates, participates in coalition efforts to promote diabetes awareness, such as the NDEP’s AAPI Working Group, and conducts its own national-level education activities, including conferences, web site outreach, and materials dissemination. NAWHO’s affiliates represent five different geographic regions – Southern and Central California, New York, Texas, and Hawaii – and serve over 20 different AAPI ethnic groups. The affiliates are:
"Four Steps to Control Your Diabetes for Life", an easy-to-read booklet for people with diabetes helps health care professionals educate patients in vital self-care principles. Written for people newly diagnosed with diabetes as well as for those living with diabetes for years, the four steps help them understand, monitor, and manage their diabetes. Presented in a simple format, 4 Steps empowers people with diabetes to be active partners in their own care and to take steps to stay healthy.
